CONFERENCE and COURSE REGISTRATION FORM
All oral and poster presentations must submit an abstract. Abstract submissions must be sent to the Conference's secretariat by email to the address firstname.lastname@example.org before 17 March 2014.
Conference Registration Fee includes proceeding of abstract, welcome cocktail, the Lagoon tour, Conference dinner, coffee breaks and VAT (value added tax). The course fee only includes the participation in all lessons, the course material, which will be distributed during the course, VAT and coffee breaks. First fill in this form and send it to us, then you will immediately receive a confirmation mail. Finally please transfer the conference and course (if any) fee to:

20 days before the event no charge/deduction, 10-20 days before the event: 50% charge/deduction, and 10 days before the event: 100 % deduction ( no refund ). Replacement delegates will be accepted with no additional charges.
